Medical Services are offered for ages newborn through geriatric.

Providing care for YOU is the focus of a Medical Home. The care provided in a Medical Home is personal, and the team’s goal is to make sure you receive the health care you need. You are also an essential member of the team. Your participation to develop your care plan is actively encouraged. What can you do to get the most out of your health care?

  • Learn about preventive care for yourself.
  • Learn about your condition, what you can do to manage it and stay as healthy as possible.
  • As best as you can, follow the plan agreed upon by you and your team.
  • Bring a list of questions to each of your appointments.
  • Always tell your team if you don’t understand something, they said.
  • Let your team know if you are receiving any care from other professionals.
  • Talk openly with your team about your experiences.
  • If you have any questions, ask!

ResourceCare patients have access to health center staff after business hours. For non-emergency health concerns/questions, please call 325-762-0669 for English or 325-762-4681 for Espanol to be directed to the on-call provider who will assess concern/question and determine whether a ResourceCare physician, dentist, or behavioral health specialist should be consulted. The provider is also able to offer clinical advice and guidance. For emergency situations, please call 911 or go to the nearest emergency room.

The 340B program was established to provide qualifying patients with affordable prescription drugs. The program is available to certain types of safety net clinics/providers. It allows these providers to stretch the limited federal resources available to them and to offer a comprehensive list of prescriptions at significantly reduced prices.
