New Patients
Your First Visit
If you are a New Patient and have scheduled an appointment, you will receive a request to complete the "New Patient Packet." Please check your email or mobile phone for this request and complete this packet as soon as possible. We can start prepping your chart ahead of time to make your visit as smooth as possible. If you have not received the packet or need to schedule your New Patient appointment please call one of our clinic locations today or use our appointment request link to request your appointment today.
- Driver’s License or Photo ID
- Insurance Card
- Proof of Residence (DL, Water or Other current Bill with your address)
- All prescriptions you are currently taking
- Immunizations Records for Children
- Proof of most current months Income for the entire household. (pay stubs must be consecutive and most current)
- If self-employed, please bring your most recent Income Tax Return
- Current Year Benefit Letter for SSI recipients
Missed medical appointments hurt us all!
You must call ahead if you’re going to miss an appointment.
ResourceCare is committed to providing quality health care to all our neighbors. When a patient misses an appointment or cancels at the last minute, we are unable to offer that time slot to another patient. To ensure we are doing all we can to provide access to care for patients, we have implemented the following:
No Show Policy
Any patient who misses an appointment three (3) times without calling in advance to reschedule or cancel, will no longer be able to schedule appointments by phone. Patients will only be able to schedule appointments in person, at the front desk of one of our locations.

ResourceCare se compromete a brindar atención médica de calidad a todos nuestros pacientes. Cuando un paciente pierde una cita o cancela al último minuto, no podemos ofrecer la cita médica a otro paciente. Para asegurarnos de que estamos haciendo todo lo posible para proporcionar acceso médico a nuestros pacientes, hemos implementado el siguiente reglamento:
Nuevo Reglamento de No Presentarse a su Cita
Cualquier paciente que pierda una cita tres (3) veces sin llamar por adelantado para reprogramar o cancelar, no podrá programar citas por teléfono. El paciente deberá programar citas en persona en la recepción de una de nuestras ubicaciones.
