What is a PCP?
A primary care provider will have a long-term relationship with you, helping you stay healthy or get better when you’re sick. Your PCP will be your partner, sharing the responsibility for your physical and mental health and wellbeing.
Primary care providers are also called general practitioners, because they cover all types of everyday health concerns. Your PCP can answer questions about infections, chronic conditions or medicines. With regular visits, such as annual health checkups, your PCP will get to know much about you:
- Your current health.
- Your medical history.
- Your family’s medical history.
- Your treatment preferences and daily challenges.
- Your personality and lifestyle.
A PCP can help you stay healthy and can be the first to treat any health problems that arise. Make time to find a PCP who is a partner in your long-term care, and have regular checkups.
A relationship with a PCP can help you detect and manage a wide range of health issues. A PCP can teach you how to lead a healthier lifestyle, provide needed medications, screen for disease and recommend specialists when you need them. Patient Centered Medical Home
We provide primary care to men and women, children, and adults. Our team of healthcare professionals is qualified to care for you through all the stages of your life.
A Patient Centered Medical Home is a medical office where a team of health care providers works together to provide care for patients. In addition to physicians, this team can include professionals such as nurse practitioners, physician assistants, nurse educators, care coordinators, social workers, and behavioral health counselors. The team works together to:
- Provide access to care 24 hours a day / 7 days a week. At ResourceCare, we are open evenings and weekends, all so we can be there when you need us. Our after-hours call service is available when the office is closed.
- Know your personal health history and family situation. This information allows providers to suggest treatment options and lifestyle changes that will make the most sense for each patient.
- Educate you about your health conditions and how you can be an active member of your health care team. By understanding all you can about your health, we empower patients to take control of their health.
- Coordinate your health services. We will help you find specialists, assist with appointments, and make sure specialists participating in your health care have all the information they need.
Utilizing evidence-based practice guidelines help providers deliver treatment that is scientifically supported clinical services that are best suited for our patient needs.
